Prior to your first visit, please read over the Policy and Procedures found under the PATIENT tab. This initial visit is considered your evaluation, which will include gathering pertinent past health information, discussing your current injury, and performing various tests and measures to help create an individualized treatment plan for your optimal rehabilitation. Please wear appropriate clothing which can expose the regions to be treated.

Most insurances may provide reimbursement of services at an out-of-network rate. A claim must be made and an invoice of PT services must be sent to your insurance provider for approval of reimbursement. Additionally, any payment you make may apply to your insurance deductible. Insurances will NOT provide reimbursement if you are using Medicare, worker’s compensation or auto insurance. We recommend contacting your insurance company about their re-imbursement rates and policies.
